MEDICAL APPOINTMENT REQUEST - please follow this link and complete the relevant request. This service is available 8am - 4pm Monday - Friday.  The team aim to respond to your appointment request within 2 hours. No log in is required.

NON-URGENT MEDICAL APPOINTMENT REQUEST - please use this link to request an appointment via 'other medical request'.  This is available from 4pm - 6.30pm.  You will get a response from a member of the team within 5 working days. Should you feel you need to be seen sooner, please wait for our full online triage to re-open at 8am (Monday - Friday), call 01526 320522 and select option 1 or contact NHS 111
