We would like to inform you of some changes here at Church Walk Surgery PATIENT CAR PARK BLOOD & TEST RESULTS
We try and accommodate your preference of clinician, but this cannot always be guaranteed. You can book an appointment by calling 01526 320522 or via this link PATIENT TRIAGE where you will be asked a set of questions regarding your medical condition. This form will be submitted and triaged by a member of our team. You will be offered an appointment based on clinical need which will either be On The Day or within 2 weeks.
All consultations are by appointment only. We have a range of clinicians and always endeavour to fit you in at a convenient time, with the clinician best suited to treat you. On the day appointments are normally reserved for those who have an urgent problem.
Please help us by cancelling appointments you no longer need or are unable to attend. You can do this by calling 01526 320522 or if you are signed up to online services you can easily cancel via SystmOnline, Airmid App or the NHS App.
Have you signed up to our SMS Text Messaging Service? This service will send you a text message a day before your appointment as a reminder. When you next visit the surgery, please speak to a member of staff for more information.
Are you able to access our Online System? You can cancel appointments 24/7 and also order your repeat medication. Please visit the surgery with Photographic ID to be issued with your username and password.

If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Please tell us:
If it is medically necessary, a doctor will visit you at home. However, in most cases, medical problems are best dealt with at the surgery where we have the necessary equipment and facilities.
If you do request a home visit, one of the doctors may ring you back to discuss the problem with you.
Please note, home visits should only be requested for medical reasons and not because of difficulties with issues such as transport or child care.
We are grateful to a number of volunteers who are sometimes available to bring patients to the surgery in their own cars provided you cover the fuel costs. They may also take you to hospital appointments to relieve the burden on ambulance services. Please ask reception staff about the availability of these services.
Enhanced access appointments are for pre-bookable non-urgent appointments. Examples of these include chronic illness, asthma checks and medicines reviews.
The service is being delivered by South Lincoln Healthcare Federation of GP practices in the area. If a patient wishes to attend an enhanced access appointment, they may need to travel to another local GP practice and may not necessarily see their own GP.
In our locality, appointments are available between 6.30pm and 8pm Monday to Friday and on Saturday and Sunday mornings. There are also a number of additional appointments being made available during core hours (8.00am to 6.30pm Monday – Friday)
