  • Church Walk Surgery is inviting you to join the Patient Participation Group (PPG). The aim of this group is to gather patients' feedback about the practice services and make suggestions about how services could be improved. The group will meet virtually due to COVID restrictions.

  • 1. Do you have any suggestions how to improve our GP practice services for you, your family or your community? Please tell us in no more than 5 lines.

  • 2. Would you like to be part of Church Walk Surgery Patient Participation Group?

  • 3. What best describes you? It can be more than one answer.

  • 4. Would you be interested in volunteering your time to be part of the working group that manages the PPG?

  • 5. If your answer to the previous question is "Yes", please tell us what skills you can offer.

  • 6. We are currently looking for a Chair of the PPG. Would you be interested in this role?

  • 7. When COVID restrictions are eased would you be interested in attending PPG meetings which are held 3-4 times a year?

  • 8. If you answered YES to question 7 would you prefer to attend the surgery for meetings or via ZOOM?
