Patient Consent
At the practice we take patient confidentiality very seriously and endeavour to preserve your confidentiality on all occasions.
Access to your records
You may wish for another person, for example a family member or carer, to be able to access all or part of your medical records. In order to allow this access we need to have your written consent on record. If you wish for another person to have access to your records please complete the Consent to Release Medical Information Form and return it to the practice.
